Happy Health’s smart ring is the first FDA-cleared wearable to measure blood pressure continuously without a cuff or calibration. The 510(k) clearance is for a device already validated for at-home obstructive sleep apnea diagnosis, offering a unified view of two major cardiovascular risks.

The FDA validation involved 850 paired measurements across 85 patients, spanning normal blood pressure through stage 2 hypertension. The study included patients across age, sex, BMI, and skin tone with no exclusions. The technology demonstrated a mean error of 0.8 mmHg for systolic and 0.1 mmHg for diastolic pressure compared to invasive arterial-line measurements.

Integrated Sleep Apnea Diagnosis Capability

The same ring enables a physician-rendered diagnosis of obstructive sleep apnea in as few as five nights. Its accuracy against in-lab polysomnography is 98%. This makes it the first and only FDA-cleared smart ring for at-home OSA diagnosis.

Sleep apnea is a leading treatable cause of high blood pressure and frequently goes undiagnosed. The ring captures over 2.8 million medical data points each night across sleep, respiratory, cardiovascular, neurological, and metabolic systems. It can now monitor both conditions continuously.

Clinical Validation and Physician Network Integration

Data from the ring is interpreted by board-certified physicians within Happy Health’s 50-state network to support early intervention. The company’s chief medical officer, Jeffrey Durmer, MD, PhD, emphasized the link between the two conditions. "Sleep apnea is one of the most common treatable causes of high blood pressure, and it goes undiagnosed in millions of people," Durmer said.

The device uses artificial intelligence to build a personalized health model for each user. This data is then routed to the physician network for clinical review and actionable insights.

Context in the Wearable Health Market

The clearance marks a significant shift from wellness trackers to FDA-reviewed medical features. Every previously cleared wearable blood pressure technology required calibration with a traditional cuff. This ring requires none.

Other companies are developing similar technologies. WHOOP has previous FDA clearance for a wearable. Oura, Apple, and Samsung are working on optical sensors for blood pressure. Huawei’s mini-cuff watch has a CE Mark in Europe, while Hilo and Signal Ring have debuted in the United States. Sky Labs has integrated its ring with South Korean healthcare.

Research shows the value of nighttime monitoring. A large observational cohort analysis found nighttime systolic blood pressure was nearly six times more informative than clinic readings for predicting mortality.

Dustin Freckleton, MD, likened the development to a major shift in chronic disease management. "This is blood pressure’s CGM moment," he said. The Happy Ring enables ongoing blood pressure measurements across day and night without any need for a blood pressure cuff or external calibration.
